I think the main takeaway from this case is never to underestimate the extent of damage under a shadow or stain even if it looks innocent enough from the surface. The distal crack which led to the decay was most likely as a result of a heavy occlusal contact as the opposing second molar was tilted resulting in one heavy occlusal contact point on the distal marginal ridge. Throw bruxism into the mix & here you areā€¦šŸ˜ . I just about held onto the mesiopalatal cusp- it was nice and chunky so it would have been a shame to remove, although it would have made the prep much easier!! I always remove the old filling & caries/cracks before I even considering removing cusps. Cutting the entire occlusal surface before removing any old dentistry is too aggressive IMHOšŸ«£šŸ«£ . For the eagle eyed šŸ¦…šŸ‘€out there, the profile of the distal of UL5 was very poor but the patient passed on changing it at this time so I recontoured it & itā€™s next on the to do listā˜ŗļø .
